Care Co-Ordinator Projects

One way that, as a Network, we are supporting patients, is to run community health checks in accessible community venues across Burnley West.

If you are over 45, you can access one of our health checks where you will see one of our Nursing Associates and will get your blood pressure, pulse and weight checked.  You will also be able to discuss any other needs you might have and our team will look into ways you can best be supported.
